Overview
Valley Senior Living is seeking a dedicated Care Coordinator to join our team. This role is ideal for a registered nurse who brings both nursing expertise and a strong ability to coordinate resident care. Qualifications
To be considered, candidates must be a graduate of an approved RN program with a current North Dakota RN license or temporary permit. A BSN, certification in Gerontological Nursing, or credentialing as a Resident Assessment Coordinator is preferred. We value experience in geriatric nursing, MDS and care planning, charge nurse or supervisory roles, and familiarity with CMS guidelines.
Training and Requirements
Our Care Coordinators receive on-the-job training, participate in ongoing education, and maintain CPR certification, ensuring continued professional development in line with Valley Senior Living’s standards of excellence.
Essential Responsibilities
The Care Coordinator will:
Assess, plan, and coordinate resident care to ensure quality outcomes.
Oversee compliance with Medicare, CMS, and facility standards.
Conduct and participate in interdisciplinary team meetings with residents, families, and staff.
Lead and monitor care planning, documentation, and reimbursement processes.
Support staff through education, guidance, and collaboration.
